Volunteer Agreement Form:

Please read before signing, as this constitutes the agreement as a volunteer and the understanding of your working relationship as a volunteer with Youth Rebuilding New Orleans (YRNO). Thank you for choosing to participate in activities of YRNO.

I, the undersigned, acknowledge and state the following:

I have chosen to travel and to perform clean-up and other work for debris and damage caused by a disaster.

I understand that this travel and work entails a risk of physical injury and often involves hard physical labor, heavy lifting and other strenuous activity; that these activities will take place on streets that are open to vehicular traffic; that the debris I will collect is of unknown origin and may be hazardous to my health; and that I will be working closely with other volunteers which may result in accident or injury caused by another participant in YRNO’s activities. I certify that I am in good health and physically able to perform this type of work.

I understand that engaging in this activity involves certain risks, not all of which are foreseen. I am engaging in this project at my own risk. I understand that this is a “grass roots” activity to clean up New Orleans area from unsightly debris caused by Katrina and its aftermath. I assume all risk and responsibility for any damage or injury to my property or any personal injury, which I may sustain while involved in this project, and related medical costs and expenses.

If I suffer any accident or injury, I authorize any other participant in YRNO’s activities to administer first aid, treatment of service to assist me. I understand that aid, treatment of service will be provided at my sole risk without any liability or risk to YRNO or the aid provider.

By my signature, for myself, my estate, and my heirs, I hereby release and discharge, and agree to defend, indemnify and forever hold harmless YRNO and its organizers, and volunteers from any and all causes of action and damages arising from or relating to my participation in this project, including but not limited to claims for personal injury, sickness or loss of limb or life, even if said claims arise from injuries or illnesses caused by the sole negligence or fault of those hereby released.

I may choose to participate in additional YRNO activities on multiple or later dates, this volunteer agreement and release will apply to any group activities in which I participate in the future.

Youth Rebuilding New Orleans (YRNO) is a non-profit corporation open to all public, private, and parochial high school students and run by a student leadership council.  Our overall purpose is to be a facilitator for local youth to become involved in the recovery and rebirth of the New Orleans area. As the future leaders of this city, region, and state, it is we the youth who must act now to bring about a better quality of life for our future.
